Community Living Alliance in Madison, WI is a community based not-for-profit organization that provides quality, individualized in home support services for people with physical or developmental disabilities and older adults in Dane County. Community Living Alliance is dedicated to empowering those we serve to live independently and with dignity, partnering with you to acquire the Personal Care, Chore Services or Service Coordination you need to promote quality of life and self-determination. If you are eligible for Wisconsin Medicaid (Medical Assistance) or wish to pay for services on your own, Community Living Alliance is here to help.

Private Pay Chore Services
Private Pay Personal Care Services
Private Pay Service Coordination

Services:

Community Living Alliance will work closely with you to develop an individualized plan to meet your unique needs. Services could include:

Housekeeping
Meal preparation
Grocery shopping
Laundry
Medication reminders, blood sugar monitoring
Companionship services
Personal care services including bathing, personal hygiene, assistance in bathroom, dressing, grooming
Accompany to appointments
Assistance with exercise regimens and range of motion exercises
Service coordination
On call personal care services seven days per week
Other supportive home care or personal care services based on your needs

You can expect quality services provided in a timely manner by caring and competent staff.

We accept Wisconsin Medicaid and have long term care insurance and private pay options.
